Community services
Lewisham and Greenwich NHS Trust provides community services in the borough of Lewisham. In the Royal Borough of Greenwich, community services are provided by Oxleas NHS Foundation Trust.
We provide a wide range of adult and childrens services in the community including district nursing, health visiting, school nursing, physiotherapy and speech and language therapy. We care for people from birth to the end of life.
Our community health teams play a vital role in supporting people with complex health and care needs to live independently in their own home for as long as possible.
Our services are mainly delivered directly within patient's homes (including care homes), at health centres, children centres, schools, GP surgeries and community hospitals.
